The surgery of abdominal hernias

The advent of laparoscopic surgery has made it possible to perform abdominal hernia repair surgeries through this new surgical space as well.

The repair methods of true abdominal hernias(umbilical hernia, epigastric hernia, laparoceles...) are different from those of inguinal hernias. 

In entrambi i casi, tuttavia, l’intervento viene eseguito in anestesia generale e prevede una notte di ricovero.
Nel caso delle ernie ventrali la rete deve essere costituita da un materiale che impedisca la formazione di aderenze tra la rete stessa e i visceri addominali. Ne esistono di molti tipi diversi, tuttavia quelle che noi prediligiamo sono realizzate con un sottilissimo strato di polipropilene, un materiale plastico molto resistente, che garantisce la tenuta della riparazione, ricoperto da un altrettanto sottile strato di ePTFE, altro materiale sintetico che ha rivelato possedere una buona azione antiaderente.

Abdominal hernias and laparoscopy: a gold standard...

The laparoscopic repair technique for abdominal hernias is known as IPOM (Intra Peritoneal Onlay Mesh): once the defect is identified, it is covered with mesh, which is secured with some resorbable staples. The end result is excellent from both a functional and aesthetic point of view, so much so that laparoscopy in abdominal hernia surgery is now considered a gold standard. Overall, the patient is left with 2 scars of 12 mm and one of 5 mm.

Inguinal hernia

Laparoscopic repair of inguinal hernia is more complex than that of other abdominal wall hernias, and requires a perfectly trained team with detailed -anatomical knowledge .

There are different techniques (TAPP, TEP, IPOM...); the one we perform, the TAPP (Trans Abdominal Pre Peritoneal), provides access into the abdominal cavity and preparation of a "flap" (or flap) peritoneal, through which to return to the abdomen the  abdominal hernias, inguinal herniahernial sac and place the mesh, which, again, is fixed with resorbable clips. The procedure, in addition to having precise indications (it is the gold standard in bilateral inguinal hernias, recurrences, and hernias associated with other pathologies) also has contraindications: for example, it is not appropriate to perform it in large hernias, for which it is better to choose other techniques.
